PAYMENT POLICY

Our providers participate in most health insurance plans in the region.  Be sure to let us know about all insurance plans you may have. If your plan requires a referral, you must obtain the referral prior to your appointment, or your appointment may need to be rescheduled until it is received.  We do NOT participate with Medicaid plans regardless if you are a new or an established patient.  We are not accepting Workers Comp or MVA patients at this time. Please have your insurance card(s), driver's license and copay ready when you check in.  If we participate with your insurance, we will bill your insurance directly.  You will be responsible for any costs not covered by your insurance, including, but not limited to, deductibles, copayments and coinsurance.  Copayments are due at the time of check in.  Deductible and/or coinsurance may also be due at check in.  If you have any questions regarding participation with your insurance, please contact our office.
